Crew Medical Services
If you are needing to renew your Medical Documentation - Click here to download the Physical Examination Information form.
Once the medical exam is complete, Crew Members must log in to the SeaCare Crew Health Portal to upload their documents for review. Review this guide for instructions on how to set up a new account.
Please note: Physicians or clinics who are already a SeaCare provider are able to upload documents on the Crew Member’s behalf to the Sea Care Provider Portal.
